Removal advice from Dr. Denton:  "The advice I have for removing, especially a long time in-place screw, is to first seat the screwdriver into the head; -- then a gentle axial tap on the screwdriver that sometimes breaks up the cold welding or bone ingrowth -- then next turn the screw slightly to the RIGHT for the same reason, -- to break up its purchase, -- then turn the screw to the LEFT to remove it.
